We offer a primary plan, benefiting anyone with standard dental care and a Periodontal Plan for individuals with a diagnosis of gum disease that requires more frequent visits.

MembershipGood oral hygiene is important and we don’t want your teeth and gums to suffer because you don’t have dental insurance. That’s why we have designed two in-house dental plans to give individuals and families without dental insurance an affordable solution.The In-House Dental Plan is an annual reduced fee dental plan that allows individuals and families to receive quality dental services from David Crumpton, DDS, at a discounted price. This is not an insurance plan and cannot be combined with any dental plan.

Once membership is paid, the member will be entitled to the discounted fee for dental services for up to 12 months. Members, spouses, dependent children under the age of 19 or full time students up to 23 years of age are eligible. Members have the right to cancel at any time by submitting a written request to the dental practice.

Plan OverviewsStandard Plan – $330 individual; $300 each additional family member

• 2 routine cleanings• 1 routine exam• 1 set of diagnostic X-rays• 2 fluoride treatments until age 19• 15% discount on all restorative services• 5% discount on automatic annual renewal

Periodontal Plan – $630 individual

• 3-4 periodontal maintenance therapy• 2 periodic exams• 1 set of diagnostic X-rays• 1 emergency exam (if needed)• 15% discount on all restorative services• 5% discount on automatic renewal
